We are pleased to offer a discussion guide to accompany a modernized template (and sample) membership application form that takes into account our evolving understanding of best practices. We have tried to strike a balance between gathering information that co-ops need, with privacy concerns of both applicants and application reviewers.

Gathering the information to evaluate a potential member through an application form is one critical component of member selection, but most co-ops also really value the information obtained through an interview process.
